also fix some stuff I missed when adding NIGHTMARE, because it only affected the other difficulties. This should be equivalent to cooljeanius/A_New_Order@f129fde, plus the addition of a new image that it looks like I forgot to copy over previously.

This is a beginning of an attempt at working on issue #57. I was just going to mark it as a draft at first, but then when testing it, I discovered that I also needed to fix some stuff I missed when adding NIGHTMARE in #53, because it only affected the other difficulties. So, because that fix is a bit more urgent, I'm not marking it as a draft after all.
