includes changes from this branch which has some changes to match MoH Kenya stuff, and adding a document_metadata table to index couchdb->type
there will be more changes to base models from MoH KE, but this is a good 'checkpoint' to merge back to main and start further changes from a new branch.
and this which is almost exactly copy pasted (only change is it clears the dbt_results table before inserting new results so only the last run is saved)
