Merge MFA UI changes #6

This PR adds some UI changes for facilitating MFA. The following features have been added:

MFA token is automatically requested when a new user is created
There's now a checkbox to allow users to request a new MFA token when they edit their profile
The MFA setup information is provided in a "Setup Information" box on the main profile page that they must click to present.
