Performance improvements

This release marks a significant performance improvement and fixes a bug where in some cases the middleware performance could degrade over time. We have also:

• Improved .NET Standard 2.0 compatibility
• Thinned out the dependency graph
• Improved request localisation middleware so that it now automatically caters for applications served from a virtual directory.
