Add issue form for submitting new issues:
* add issue form for bugs with:
- Field for describe the bug;
- Field for instructions of how to reproduce the bug;
- Dropdown to select the channel used;
- Dropdown to describe the environment where the bug happened;
- Field to add relevant log output;
- field to add additional content.
* add issue for for features request:
- field to explain what is needed;
- field to explain why is needed.
