Pathology tests on physician form is edited in accordance ranjpur pathology form.

@hup-en Oh, thank you, but the code you deleted includes necessary parts which must not be deleted:
for example, patient. ethnicity_code, patient.district, and so on.
Could you share the background of this change and then let us think a proper way to merge the change to the existing necessary fields.
