- removed some unused variables
- updated documentation
- remove a few more obsolete macros
- fixes effective central bin size in the Interfile header #1034
removed *NAMESPACE_STD, STIR_SPEED_UP_STD_COPY and STIR_ASSERT All were essentially unused anyway
ran git-fame.sh again
This keyword only makes sense for arc-corrected data. Therefore, we now only read/write it in that case. Fixes UCL#1034
