Custom Alerts for MyAlerts 1.1.1

This release has been packed with a completely rewritten approach to the alert generation in the ACP dedicated module to ensure no SQL errors whilst creating a new alert and of course MyAlerts 1.04+ compatibility. The "forced" setting has been changed to a On/Off radio for comfort and a new option which lets you choose the sender of the alert has been added.

Last but not least, performances has been boosted up too alongside my other MyAlerts-related addons.
