Summary

In the New Patient Form, after successful creation of a patient, ideally, the form values should be reset. However, since the values are not resetting, it triggers the handleChange function, which then evaluates to an error notification as if the values are already in use.

In this PR, after submitting the form, it clears the form values and also sets the max allowed date for the Date of Birth field to the current date.
