Acceleration Gcode Generator failure [bug]

I've used the site successfully before, but today when I attempted to generate acceleration test gcode it failed to properly generate. Looking at the output gcode file, all it contains is:

; custom start gcode
START_PRINTundefined; custom end gcode
END_PRINT

This is after filling in all variables correctly, to the best of my knowledge. Please see attached generated gcode, gcode settings output, and screenshot of the page in question. It wouldn't let me upload the actual gcode file so I converted it to a txt file with no other changes. Thanks.

Related to this issue. In my case I was using a .3 layer height, switched to .2 and I was able to generate the gcode for the test.
