Smaily subscription form enhancement

Additional features to add to Smaily subscription form

*Under widget settings admin can change placeholder name.
*Screens with resolution under 450px should have input field width 100%. Screens 450px and upper should have input fields with max width 450px.

Name input is already optional, by default the form only has an email field active.
All the form styling is inherited from the page. Adding custom styling will work for specific sites, but might break things on another. I think it should be managed by the page owner or theme.

Additional features that could be added to Smaily subscription form:

• Option to change button text in widget.
• Option to choose form layouts (like in WooCommerce widget).
• Option to change button and button text colors (like in WooCommerce widget).
