Notes Title to be included in the chunk. Provide clear separations between notes for LLM to distinguish

Hi Kudos for Reor team, this is a dream to have built in functionality for Notes app

My humble suggestion after trying and testing, the LLM often jumble all the chunk context for their answer.
I'm using 2 notes as filter to pinpoint the issues on the chat and try to summarize each notes separately and as the chunk is not having informations from which notes it come from it cannot really distinguish each notes summarization.

I'm sorry if this already implemented but currently I'm seems to failed to get accurate response when adding several notes as context. I'm using Ollama with hermes2pro-llama 3 and Dolphin-llama3 2.9

I think this basic functionality will enhance the current Reor (i'm using 0.2.8) make it more price and accurate as Local RAG main selling point.
