In the releases page:

  1. release v4.0.3 is assigned tag v4.0.2
  2. tag 4.0.3 is missing, so there are a lot of changes not documented (4.0.3...v4.0.4)
  3. release v4.0.4 is not the same as branch 4.0.4. as there are 4 commits missing (v4.0.4...4.0.4)
