Problem adding Categories to existing items

What steps will reproduce the problem?
1. categories not created when i first create an item, enter the information 
and then it does not actually create the category
2. trying to add a category after the fact to an existing item, works 
sometimes, not others
3.

What is the expected output? What do you see instead?


What version of the product are you using? On what operating system?


Please provide any additional information below.
this problem has been going on for a few weeks. also a problem with updating 
quickly when a new item is created. have to leave the page and come back for 
the item to appear.

We have been able to reproduce the issue with item creation, and have logged it 
as a bug.
Can you provide more information about where in the site you are adding 
categories and having difficulties?
