Seems the primary monitor is working fine, but any other monitors seem to operate as normal. Would you have a quick way around this?

Curious, on my machine it does work the way you would expect on all three monitors.
What does ps -aux | grep alternating_layouts.py show? When and how do you start it?

I can confirm that this works as expected on a dual monitor setup - Debian Bullseye with i3. It is a laptop with external USB-C monitor attached.
I had the same issue. Rebooting fixed it.
