Where an authority has a home page URL of the format https://www.gov.uk/government/organisations/XXX, set the disclosure log (if empty) to https://www.gov.uk/search/transparency-and-freedom-of-information-releases?content_store_document_type=foi_release&organisations%5B%5D=XXX.
I did ponder if there'd be a benefit to doing this before.
However, anecdotal evidence suggests that many of these logs on gov.uk aren't kept up-to-date (for example: DCMS last updated theirs in 2017, and DVLA's one hasn't been changed since 2016). The Cabinet Office's version is notable, as they manage a 6 year gap between 2015 and 2021 - with only one entry in that period.
Notably, many organisations don't use it at all (for example: the CMA, and RAIB) - which is poor.
If there's minimal effort required though, it might not be a bad idea.
