Notifcations: Duplicated entries for users with multipe roles in the same course

Describe the bug

I happen to be in three user groups (tutor, editor, and instructor) for the same course. This leads to notifications being showed three times in the notification bar:

The same error happens with announcement-e-mail, here I also get every announcement three times.

To Reproduce

1. Add yourself to multipe usergroups of the same course.
2. Trigger a notification e.g. by asking a question or relaying.
3. See duplicated notifications
