Reorganize steps in the protocol based on purpose #116

There are two arbitrarily separated steps in the protocol:

Demographics
Confounders

We can rework this to be based on the purpose of the question and when it will be asked.

Demographics -> questions about the patient that we only need to ask once
Confounders -> questions about the patient we want to ask right before they give voice samples
