Describe the bug
Legal Record Management form is not looking legal XD,
I can fix that, I am familiar with UI/UX

To Reproduce
Steps to reproduce the behavior:

Go to 'https://noplaybutton-41656.web.app/'
Click on 'https://npb-e-vault.vercel.app/'
UI not proper

Expected behavior
A well organized and structured form matching with the website, for better UX
