Files get replaced when adding a file

When I'm adding a new file it sometimes replaces some of the existing ones, so that the newer file appears under the name of that old one (and the old file gets deleted).

Steps to reproduce the behavior:

1. Go to 'Edit tool'
2. Click on 'Upload files'
3. Upload a file
4. Save the tool

Sometimes it also resets the settings for the other files (like if the file is hidden).

Expected that the file is added as a new one, and all other files aren't get touched.

This error doesn't always appear. Sometimes it works as expected.
