How to debug a custom plugin?

I'm writing my own custom plugin according to https://heyapi.dev/openapi-ts/plugins.html#custom. How can I debug handle function in webstorm or vs code?

I see that handler is being called now, but for some reason the file is not being created

If the file doesn't contain any nodes (as in your example), it won't be generated. Try putting something inside with file.add(nodes)
