When deleting the hotspot (from ContentArea) it is then still visible on the list of hotspots generated on a page. The probable reason behind it is that list of hotspots is serialized (and saved afterwards) only if user manipulates the location of the hotspots on a preview image.
Steps to reproduce:
Setup Image Hot Spot by defining ie 5 hotspots
Publish & see the page with hotspots
See that there are 5 hotspots rendered
Remove one hotspot from ContentArea
Publish the page
See that there are still 5 hotspots
You can modify these steps, by repeating steps 1-4. Then do:
5. Modify position of any of hotspots on the preview image
6. Publish the page
In modified scenario you will see 4 hotspots
