The intention is to import the ldap module provided by the "python-ldap" package. On Ubuntu 14.04, though, the module itself (i.e. the authenticator) is imported instead, leading to this error:
'module' object has no attribute 'initialize'
The same issue applies to the fallback import in the pam.py authenticator.
