I really need to add skins and heads as items and not just unlock them.

Out of curiosity, what's your use-case for adding skins as items versus unlocking them?

I'm on Xbox so there's no way of editing save files. If I could add skins and heads as items to my friend's (PC player) inventory he could simply give me these items in-game.
