• Type of Merchandise (Dropdown?)
• Store / Portfolio URL
• Sold at CPAC before? (Y/N)
• Can you sell during all hours? (Y/No and provide specific times)
• Number of spaces
• Number of corners
• Number of Tables
• Number of Chairs
• Number of Badges
• Selling Food? (Y/N)
• Electricity Required? (Y/N)
• Sign Contract
• Agree to Policy
• Additional Comments
