Incident creation form attribute values do not reflect updated attribute order #1033

Repro

Navigate to the Manage pane of a project.
Reorder several attributes into several groups.

Expected
Order should update everywhere, including on the new incident page.

Current
The attribute names update, but are attached to the wrong attribute values.
