Create a module that can be used by any backend service to create notifications for users. It must be very well encapsulated, flexible, easy to extend, easy to use and support real-time operations.

It's open to discussion, but I think it'd be nice to use technologies like WebSockets and Redis to enable fast and real-time communication.
