There has been an uptake in users having cases where there are duplicate entries in SP Reg.
Originally we thought that the core problem would be a double clicking the button.
However, recently in REQ7158888 Andrew noted:
"I noticed that the two records were inserted into the database within 20ms of each other. This looks less like a user issue (user pushing the submit button twice) and more like an application issue where the app, for some unknown reason, executed two consecutive writes. I've seen this sort of thing before. It would probably take some significant research to determine how this happens and therefore how to prevent it."
