We had an issue come in from a discourse about some functionaity from the file picker not working right in TensorBoard after upgrading to 3.0.

I think this is a bug against this repo: https://github.com/OSC/bc_js_filepicker.

I no longer see this issue after updating form.js from v0.4.1 to the latest v3.0.2
