• Skipto the breastfeeding question in the form if the patient is not pregnant

Currently follow up questions are asked

• Allow user specify if other danger sign is selected
  

• if the answer to the question Is the patient Breastfeeding? is NO, then the next question on Is the breast milk enough should not be shown

@mathewwahome , kindly confirm the form is submitted for a specific patient.

In the below example, I'm accessing patient named Akama Junior who is the household head as shown below:

But the end report submitted picks the name of the CHP, currently named Felister Kirarei

The expectation is that the report is linked to Akama Junior and should be listed here:
