In addition to encounter data, what other info can we find?

Diagnosis

  • most responsible: the one that accounts for the largest portion of stay
  • Discharge Abstract Database (DAD)
  • every acute care encounter has a DAD
  • every time you are admitted to a hospital, you get a DAD
  • but not in residential care, or any other secondary/tertiary services
  • coded using ICD-10 (international classification code)
  • length of stay (which is not a proxy for quality of services)
  • ALC status: alternative level of care (e.g. some facility after discharge)
  • DAD contains procedures (coded)

Labs

  • community labs
  • Island Health labs

PharmaNet

  • BC database, not national
  • makes BC distinctive

NACRS

  • coding the reason for being in ED

RAI

  • see the example of the ASSESSMENT FORM
  • Resident Assessment Instrument
  • 12 different versions of RAI (e.g. RAI-HC, RAI-MH, RAI-CMH, etc)
  • activities of daily living

MRR

  • Minimum Reporting Requirement
  • required for all MHSU services + Psychogeriatrics
  • HONOS ( a simplified mental status exam)
  • contains substance use profile (e.g. number of uses in the past 30 days)
  • clinician is asked to provide an opinion (not a formal diagnosis)
  • is NOT used by physicians, but by service planners

MSP

  • primary care data
  • Physician's billing data
  • Records the reason for seeing a doctor
  • tells you when a patient had an encounter with a physician
  • diagnostic information is not clean and often vague
  • includes general physicians as well as specialists

Billing data

  • health authority actuarial data

Regarding Diagnosis section:
•every acute care encounter has a DAD - Eventually. For hospitals not using the electronic health record functionality, this is entered manually and as a result, can be delayed up to a few months after discharge.
•every time you are admitted to a hospital, you get a DAD - Specifically, when you are discharged
