Reproduction:

Using a freshly printed health analyzer, i somehow managed to get the old brute-toxin-oxy-burn damage values instead of the updated UI. The patient was irradiated at the time, if that helps

To my knowledge the only way to get this is by being clumsy or brain damaged, the 'whoops you fucked up and scanned the floor' UI is hardcoded as part of /obj/item/healthanalyzer/attack()
