Create user-friendly public-facing version of the the data entry-screen #13

This would be a public option to get more input without Code for Pittsburgh doing the data entry. This should include option to upload (csv,txt,pdf). These files would have to be verified before updating the db.

I still want to make a public version of the form. Basically, thinking it is login-less and has everything except for the validation, publication, and delete buttons--the "data curation team" can be responsible for scrubbing those submissions.

I'll pass on the file upload portion now that we have a ReST API in place for programmatic data submission.
