Erase form - Not working

Erase form - Not working when using LocalStorage (Firefox 43). It does clear the cookie on WP8-IE.

Not working in Chrome (i didnt tested other browsers).
It doesn't work because there is a bad check about the existance of "$.cookie"
To fix it, include https://github.com/carhartl/jquery-cookie (no more mantained)
