If you mean proxy's destination IP, that's just a Cloudflare IP. When you run this on a worker and create the proxy as said in the README file, Cloudflare knows which worker you need to work with by reading the SNI from your proxy packets. You just need to assign a Cloudflare IP (or your worker's domain, no difference) as the dest. IP to tell your proxy client to send the packets to Cloudflare's network.
